Vasospastic reactions are considered to be one of the risk
factors in the pathogenesis of glaucoma and they may lead to visual field defects via
disturbing autoregulatory mechanisms of blood flow in the optic nerve head. Endothelin-1
(ET-1) is the most potent vasospastic mediator, produced by vascular endothelium, which is
responsible for autoregulation. In this study we measured ET-1 plasma levels in basal
conditions and after cold-pressor test in three groups of subjects: 1) primary openangle,
high-tension glaucoma patients, 2) normal-tension glaucoma patients, 3) healthy persons,
and tried to correlate changes in ET-1 plasma levels with static perimetry results after
this test. Significant elevation of ET-1 plasma levels occurred in all the three groups
examined after cold-pressor test. This elevation was significantly highest in
normal-tension glaucoma patients and associated with significant decrease of mean
sensivity (MS) value in the static perimetry. Results of the study indicate that ET-1 may
be involved in the vasospastic reactions, provoked by cold, and these reactions are
strongest in normal-tension glaucoma patients. The results also suggest, that vasospastic
effects of ET-1 may lead to visual field damage in normal-tension glaucoma patients. |
