patients with cataract and pseudoexfoliation glaucoma

Purpose of the study:
To compare the results obtained after
phacotrabeculectomy with use of iris retractors and
implantation of intraocular lens in patients with
cataract and pseudoexfoliation glaucoma.
Patients and methods: The retrospective study
covered 14 patients (14 eyes) at the average age of
73±6.31 years, with cataract and pseudoexfoliation
glaucoma with IV PEX. Due to poor mydriasis resulting
from pseudoexfoliation syndrome (PEX) course, in all
patients it was necessary to apply iris retractors in
order to enlarge the pupil. The opacified lenses were
removed with phacoemulsification method. Trabeculum was
removed with punch method. A foldable intraocular lens
was implanted inside the capsule. Visual acuity,
intraocular pressure and the ratio of antiglaucoma drugs
were subject to assessment.
Results: The observation period was 12 months.
The average visual acuity prior the surgery was 0.15 +
0.13, while after a year post surgery it was 0.59 + 0.37
(p < 0.001). However, we observed decrease in
intraocular pressure from 22.6±5.42 mmHg before the
surgery to 18.7±2.87 mmHg (p < 0.001) after surgery. The
anti-glaucoma medication ratio was also reduced from 2.2
to 0.2 in the post-surgical period. Inflammatory
reaction of the anterior chamber was observed in 4 eyes.
Conclusions: Application of iris retractors
during cataract and glaucoma surgery in patients with
pseudoexfoliation syndrome considerably facilitates the
implantation procedure of an intraocular lens into the
capsule. Phacotrabeculectomy with iris retractors is an
efficient and safe surgical method, to be recommended
for patients with cataract and pseudoexfoliation
glaucoma. |
