Summary: The investigations were carried out to determine the role (relationship) between cigarettes smoking and incidence of new lens opacitates. Study conducted in Medical University Gdansk from 1998 to 2000 year. Analysis included 401 individuals (age 70 years in 1998 year). They were free of lenses opacities at the first eye examination. I devided my patients into two groups:
group I – smokers 288 patients,
group II non- smokers – 113 patients.
Data on the smoking history of the subjects were collected by personal interviews that were conducted in 1998 and updated in 2000 year. Lenses were determined by grading standard slit lamp and retroillumination photographs of the lens baseline and 3-years follow-up examinations. Lens photographs were taken and graded for presence and severity of cortical, nuclear and subcapsular cataract. During 3 – years follow-up cataract incidence among 100% participans – smokers were confirmed. Among the 100% eyes with cataract (smokers only), nuclear cataract was present in 65.9%. Subcapsular changes were observed in 13.6% smokers. These data provide support for the hypothesis that cigarette smoking increases the risk of nuclear cataract development. Compared with never smokers, current smokers of 25 or more cigarettes per day had a statistically significant increase in the risk of cataract. The significant relationship of smoking with lens opacities suggest an etiologic effect but we need further investigations.

Key words: cataract, cigarette smoking.
