Active Prophylaxis Role in Early Diagnosis of the Glaucomatous Neuropathy

Summary: Aim of this paper was to evaluate how individuals with suspected, but not confirmed glaucoma during the first observation, perform indicated follow-up visits and how often glaucoma is diagnosed in this group later on.
Material and methods: Examined group consisted of 50 individuals with suspected glaucoma (42 F and 8 M) with no evidence of glaucomatous damage confirming glaucoma diagnosis at the time of the first observation. Mean follow-up time was 4 years. Compliance with the recommended follow-up regimen was evaluated and complete ophthalmologic examination including intraocular pressure and static perimetry was performed. Results were compared in 3 groups: I – eyes without glaucoma, II – eyes with glaucoma confirmed during earlier follow–up visits and III – eyes with glaucoma diagnosed on present examination. Diagnosis was made according to the European Glaucoma Society criteria.
Results: Only 18% of patients followed the control examinations schedule, 32% visited ophthalmologist due to different eye problems and 50% remained uncontrolled. Glaucoma was confirmed in 38% of patients. IOP was significantly higher in the III group (24.2 mmHg) comparing to the groups I (16.2 mmHg) and II (13.7 mmHg). The most severe visual field defects were observed in group III.
Conclusions: 1. Most of the individuals with suspected glaucoma remained uncontrolled after the first observation. 2. A special attention should be paid to inform patients properly about the disease and active prophylaxis should be considered in this group.
