Inferior Transposition of the Oblique Anterior Muscle in the Treatment of the Inferior Oblique Muscle Overaction

Summary: The aim of the study was to determine whether inferior transposition of the oblique anterior muscle is beneficial for treatment of the inferior oblique muscle overaction.
Material and methods: 15 children (6 girls and 9 boys) aged 4 – 14 years (mean 9.1 years) with unilateral +3 or +4 overacting inferior oblique muscle underwent unilateral inferior transposition of the oblique anterior muscle between 1999 and 2003. 9 patients had horizontal strabismus: 8 children had esotropia from 18 to 60 prism dioptres and 1 child was exotropic with 30 prism dioptres. Inferior oblique muscle was placed at the temporal border of the inferior rectus muscle.
Results: Residual overaction was observed in 4 cases (26.7%). The inferior oblique action became close to the normal after anterior transposition in 11 eyes (73.3%).
Conclusions: inferior transposition of the oblique anterior is an effective and save method of treatment in the inferior oblique muscle overaction.
