Posterior ischemic optic
neuropathy (PION) is a rare condition that follows
infarction of the posterior portion of the optic nerve.
This disorder can be divided into three distinct
subtypes based on the clinical scenario and associated
conditions. The most frequent type - nonarteritic PION,
has been described in association with a variety of
conditions including atherosclerosis, arterial
hypertension, diabetes and nocturnal hypotension.
Arteritic PION can occur with a systemic vasculitis,
most commonly giant cell arteritis. Perioperative type
of PION occurs in the setting of profound hypotension
and severe anemia during the perioperative period.
Clinical features of posterior ischemic optic neuropathy
include sudden visual loss, variable visual field
defects, relative afferent papillary defect and normal
optic disc. Diagnosis of PION can be made only after
other causes of the optic neuropathy have been excluded. |
