This article compares a
management strategies in the diagnosis and a modern
therapeutical approach to the macular holes treatment.
Author presents a question problem from the first report
to the present days. He adverts on essence of macular
hole formation and it's therapeutic implications. He
presents a gradation of macular holes based on Gass
scale and a qualification system for the surgical
management. He adverts on differentiation which
sometimes cause difficulties for the ophthalmologists in
their ambulatory work. It considers a surgical treatment
possibility as only right procedure by perform a pars
plana vitrectomy, which is effective but difficult, not
without complication. It describes the operation
technique in details, including internal limiting
membrane (ILM) removal with descriptions and operation
photos. At the end there is a results comparison with a
qualification scale in relation to patient's visual
acuity improvement treated by a vitrectomy through the
ciliary body. It emphasized that a new technical
achievements will improve surgical results in comparison
to former reports on this topic. |
