Confocal Microscopy of Cornea in Iridocorneal Endothelial Syndrome

Summary: Aim: To describe corneal confocal microscopy findings in cases of iridocorneal endothelial syndrome (ICE).
Methods: 4 patients (3 men and 1 woman) aged 48-71 years (mean 56) were diagnosed. The ophthalmic examination was carried out including visual acuity, slit lamp biomicroscopy, fundus examination, tonometry, gonioscopy, pachymetry and confocal microscopy of the cornea. In 2 cases anterior segment optical coherence tomography was performed.
Results: Slit lamp biomicroscopy revealed discrete, usually linear lesions on the posterior surface of the cornea. In vivo confocal microscopy imaged characteristic epithelium-like endothelial cells and stromal oedema usually limited to the area adjacent to the pathological endothelium.
Conclusions: Presented cases confirm the clinical usefulness of confocal microscopy in the evaluation of endothelial pathologies. In-vivo confocal microscopy allows imaging the specific histopathologic corneal features of ICE syndrome also in its early stage, when changes are limited to cornea.
