Relevance of the Screening for Glaucoma in Patients Treated for Arterial Hypertension

Summary: Rationale: Thessaloniki Study revealed increased cupping and decreased rim area of the optic disc in patients with diastolic blood pressure <90 mm Hg on systemic antihypertensive treatment. Other population-based studies showed association of low diastolic perfusion pressure with higher prevalence or incidence of glaucoma. Therefore it seems reasonable to screen patients with treated arterial hypertension for glaucoma.
Purpose: To screen patients with recognized and treated arterial hypertension for glaucomatous damage.
Design: Prospective cross-sectional study.
Study population: 38 consecutive patients (18 men, 20 women) with long history (at least 5 years) of diagnosed and treated arterial hypertension registered at the Hypertension Clinic at the Dept. of Cardiology Medical University of Wrocław.
Methods: Ophthalmologic data: visual acuity, gonioscopy, IOP, pachymetry, HRT II, visual filed (Octopus, dG2). Systemic data: records of blood pressure (BP) from the Hypertension Clinic (at least 4 records) or ambulatory BP monitoring, anti/hypertensive medications.
Results: Thorough ophthalmic examination revealed 6 (15.8%) glaucomatous subjects in a group of 38 consecutive patients with treated arterial hypertension. Three out of six glaucomatous subjects had normal pressure glaucoma. Mean systolic BP was 135.3 mm Hg (± 14.3), mean diastolic BP was 81.4 mm Hg (± 9.9). Mean IOP was 13.1 mmHg (± 2.5) for right eyes and 12.9 mmHg (± 2.3) for left eyes.
Conclusions: This pilot-study shows the need for a large-scale glaucoma screening project in patients with treated arterial hypertension.
