| Summary: |
To describe a case of
unilateral glaucoma following bilateral LASIK surgery.
Case report: 30 years old high myopic female without a
history of ocular hypertension underwent bilateral
uncomplicated LASIK surgery. Refractive error before
surgery in right eye was: -10.5 Dsph -0.25 cx 450, in
left eye -8.25 Dsph -0.5 cx 1800. BCVA in right and left
eye before surgery was 1.0. Intraocular pressure
(Pascal) – normal level before surgery: 13.0 mmHg (right
eye) and 14.0 mmHg (left eye). Static perimetry (HAAG-STREIT)
OCTOPUS 301 was without any glaucoma symptoms before
LASIK.
The postoperative period was complicated because of the
IOP increase induced by topical steroids. Topical
steroid therapy was discontinued after a total of 6
weeks. Ten months after LASIK the right eye gave
symptoms of glaucoma. The IOP (41 mmHg) did not return
to normal range after topical antiglaucoma therapy.
Eleven months after LASIK patients right eye underwent
trabeculectomy. There was no IOP elevation in the left
eye.
Conclusion: Steroids, typically used after refractive
surgery can increase intraocular pressure in steroid
responders. If the IOP is elevated after refractive
procedure, corticosteroid drops should be discontinued
immediately and patients need periodic IOP checking,
static perimetry and visual acuity control. |
