The Assessment of Corneal Morphology 10 Years after Photorefractive Keratectomy

Summary: Purpose: The aim of this study was to examine in-vivo corneal morphology 10 years after photorefractive keratectomy (PRK).
Material and Methods: We analyzed 96 corneas of 60 patients (20 women and 40 men in age 23-49 years), who underwent PRK for myopia and myopic astigmatism. The spherical equivalent of the refractive error before surgery ranged from -6.75 D to -2.0 D. Confocal microscopy examinations were performed before and during 10 years of follow-up.
Results: Confocal microscopy showed alterations within the epithelium and anterior part of corneal stroma characteristic for state after photoablation. In 74% of eyes blurred structures of collagen fibers, increased reflectivity were observed. In three patients in the anterior part of corneal stroma the cicatrical tissue was observed. The Bowman’s membrane was absent in the central area, but regenerated subepithelial nerve plexus was detected.
Conclusions: In-vivo, corneal morphology after PRK showed alterations within its structure still 10 years after surgery. Subclinical lesions of corneal structure detected by microscopic examination in clear corneas do not seem to affect post-PRK visual acuity.
