Measuring the Rate of Glaucoma Progression – Practical Approach

Summary: Glaucoma is a chronic, progressive disease leading to severe, irreversible visual impairment. All patients with glaucoma progress, but not at the same rate. According to new Guidelines of European Glaucoma Society rate of progression is invaluable for making clinical management decisions. Progression rate is also essential in projecting the risk for future functional impairment. Measurement of progression rate therefore needs to be determined in each individual patient to allow for the most appropriate treatment decision. Currently, progression rate can be best determined by detecting visual field changes, using standard automated perimetry (SAP). VF defects reflect functional damage, which has an impact on patient’s QoL and is most important from the patient point of view. Besides, VF is measured with stable technology and SAP is widely available in clinical practice. To evaluate progression rate, it is essential to establish good set of reliable baseline data (at least two reliable VF measurements with threshold SAP). Estimating the rate usually requires several years of frequent exams. As progression in most patients is linear, the likely treatment outcome can be projected if enough VFs are obtained. The authors present practical recommendations for measuring clinically relevant rates of glaucomatous visual field progression.
