Glaucoma Drainage Devices

Summary: Drainage devices have been applied in glaucoma surgery for over thirty years. They are most frequently used in complicated treatment of glaucoma that does not respond to medications, as secondary procedure after the failure of primary antiglaucoma surgery, after penetrating keratoplasty and following vitreo-retinal procedures. Their mechanism lies in creating alternate aqueous pathways by channeling aqueous from the anterior chamber to subconjunctival filtration space formed around flat equatorial drainage plate. Then, by passive diffusion through conjunctiva to peribulbar and lymphatic blood vessels. The described in literature effectiveness of drainage devices in the reduction of intraocular pressure, beneficial safety profile and small number of severe complications lead to their wider and wider application. Furthermore, seton implants are used by some surgeons as primary procedures.
