Acute Postoperative Endophthalmitis

Summary: Acute postoperative endophthalmitis is infectious complication often leading to vision loss. Due to the highly dynamic process the proper treatment should be initiated as soon as possible. Until recently it was limited to the use of local and general antibiotic treatment and intravitreal antibiotics injection. Vitrectomy was performed only in cases where visual acuity deteriorated to the level of fingers counting. In the course of inflammation there is often permanent retinal damage. The bacteria and bacterial toxins located near the retina are directly responsible for this condition, causing toxic, infectional and inflammatory retinal damage. In recent years there has been a concept of early total vitrectomy involving its earliest implementation, resulting in the removal of inflammatory mediators from the interior of the eye and improving medicines action. Thanks to tremendous progress in vitreoretinal surgery and the greater availability, it appears that should not be given any treatment for postponing this operation and it should be done as soon as possible, giving opportunities to improve prognosis.
